{"id":243,"date":"2023-05-31T03:42:02","date_gmt":"2023-05-31T03:42:02","guid":{"rendered":"https:\/\/ppsg.ch\/?page_id=243"},"modified":"2025-05-08T10:48:16","modified_gmt":"2025-05-08T10:48:16","slug":"behandlung-mit-antidepressiva","status":"publish","type":"page","link":"https:\/\/ppsg.ch\/?page_id=243","title":{"rendered":"Behandlung mit Antidepressiva"},"content":{"rendered":"\r\n<h3 class=\"wp-block-heading\"><span style=\"color: #467aa7; font-family: 'Open Sans';\">Die moderne Behandlung der Depression<\/span><\/h3>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<p><span style=\"font-family: 'Open Sans';\">Im Jahr 2022 waren 8% der Bev\u00f6lkerung wegen psychischen Problemen in Behandlung.\u00a0 Die Depression ist die am h\u00e4ufigsten auftretende psychische Erkrankung. 10% der Schweizer Bev\u00f6lkerung weisen mittelschwere bis schwere Depressionssymptome auf. Frauen (12%) und junge Menschen (19%) sind h\u00e4ufiger betroffen als M\u00e4nner (8%) und Personen ab 65 Jahren (4%).<sup>1 <\/sup>Auf dem Genom (Gesamtheit aller Tr\u00e4ger der Erbinformation einer Zelle, wie Chromosomen, DNA, RNA) konnte die Wissenschaft bislang 44 Genorte identifizieren, die mit schweren Depressionen in Verbindung stehen.<sup>2\u00a0 <\/sup>Epigenetische Ver\u00e4nderungen (vererbbare Chromosomen-Modifikationen ohne Ver\u00e4nderungen der DNA-Sequenz) bestimmen, ob die Depression wirklich ausbrich.<sup>3 <\/sup>Umwelteinfl\u00fcsse bewirken unter anderem solche epigenetischen Ver\u00e4nderungen. Bei einer Depression kommt es zu Stoffwechselentgleisungen im Hirn. Dass der Stoffwechselhaushalt der Hirnzellen wieder ins Gleichgewicht kommt, ist das Ziel der medikament\u00f6sen Behandlung von Depressionen. Das menschliche Gehirn hat insgesamt 8.6\u00b710<sup>10<\/sup> <a href=\"https:\/\/ppsg.ch\/?page_id=2771\"><span style=\"color: #0000ff;\">Nervenzellen<\/span><\/a> (86&#8217;000\u2019000\u2019000). Jede Nervenzelle ist \u00fcber ungef\u00e4hr 10<sup>5<\/sup> <span style=\"color: #0000ff;\"><a style=\"color: #0000ff;\" href=\"https:\/\/ppsg.ch\/?page_id=2771\">Synapsen<\/a><\/span> (100&#8217;000) mit anderen Nervenzellen verbunden. Das <span style=\"color: #0000ff;\"><a style=\"color: #0000ff;\" href=\"https:\/\/ppsg.ch\/?page_id=2771\">neuronale Netzwerk<\/a> <\/span>des Menschen besitzt so zirka 8.6\u00b710<sup>15<\/sup> (8600\u2019000\u2019000\u2019000\u2019000) synaptische Verbindungen. Die Fortleitung des Impulses in der Hirnzelle ist ein elektrischer Vorgang. Der elektrische Impuls f\u00fchrt zur Aussch\u00fcttung eines \u00dcbertr\u00e4gerstoffs oder Hirnbotenstoffs (Neurotransmitter) in den Spalt zwischen den Hirnzellen (synaptischer Spalt). Es sind \u00fcber 100 erregende und hemmende Neurotransmitter bekannt.<\/span><\/p>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<h6>\u00a0<\/h6>\r\n<figure class=\"wp-block-image\"><span style=\"font-family: 'Open Sans';\"><img decoding=\"async\" class=\"wp-image-3385\" src=\"http:\/\/www.psychiatrie-zh.ch\/wp-content\/uploads\/Abb_Hirn_Transmitter-300x169.png\" alt=\"\" \/><\/span><\/figure>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<h3 class=\"wp-block-heading\"><span style=\"color: #467aa7; font-family: 'Open Sans';\">Wirkungsweise von Antidepressiva<\/span><\/h3>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<ul class=\"has-text-color wp-block-list\">\r\n<li><span style=\"font-family: 'Open Sans';\">Effekte auf Hirnbotenstoff-Ebene und Rezeptor-Ebene, Andockstelle einer Zelle, die meistens an der der Zelloberfl\u00e4che liegt.\u00a0 Wenn sich\u00a0 Hirnbotenstoffe an den Rezeptor binden, l\u00f6sen sie ein Signal in der Zelle aus (Schl\u00fcssel=Botenstoff und Schloss=Rezeptor). Nur wenn beide zusammen passen, kommt es zur Reaktion wie Transport, Metabolismus (Abbau), Agonismus (Aktivierung) , Antagonismus (Blockierung), Modulation (Ver\u00e4nderungen)<\/span><\/li>\r\n\r\n\r\n\r\n<li><span style=\"font-family: 'Open Sans';\">Effekte auf Signal\u00fcbertragung\u00a0<\/span><\/li>\r\n\r\n\r\n\r\n<li><span style=\"font-family: 'Open Sans';\">Effekt auf Genexpression (wie ein Gen zum Ausdruck kommt und in Erscheinung tritt)<\/span><\/li>\r\n\r\n\r\n\r\n<li><span style=\"font-family: 'Open Sans';\">Effekte auf \u00fcbergeordneter Ebene des Zentralen Nerven Systems\u00a0 (Modulation der Plastizit\u00e4t des Hirns,\u00a0 Neubildung von Hirnzellen, Beschaffenheit der Rezeptoren)<\/span><\/li>\r\n\r\n\r\n\r\n<li><span style=\"font-family: 'Open Sans';\">Effekte auf Verhaltensebene (Stimmungsaufhellung, Aktivit\u00e4tssteigerung, Normalisierung der Denkst\u00f6rungen, Verminderung der St\u00f6rungen des vegetativen Nervensystems)<\/span><\/li>\r\n<\/ul>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<h3 class=\"wp-block-heading\"><span style=\"color: #467aa7; font-family: 'Open Sans';\">Funktion der Neurotransmitter (Hirnbotenstoffe)<\/span><\/h3>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<p><span style=\"font-family: 'Open Sans';\">Serotonin (5-HT), Noradrenalin (NE), Dopamin (DA), \u03b3-Aminobutters\u00e4ure (GABA), Acetylcholin (Ach), Histamin, Glutamat und andere Hirnbotenstoffe sind an der Stoffwechselst\u00f6rung im Hirn beteiligt. Auch\u00a0 St\u00f6rungen des Hormonhaushaltes (Adrenalin, k\u00f6rpereigenes Kortisol) spielen \u00a0bei der Depression eine Rolle. Mehr als die H\u00e4lfte aller Synapsen im Gehirn verwenden Glutamat als Transmitter. N-Methyl-D-Aspartat Antagonisten bzw. \u00abGlutamat Rezeptor Blocker\u00bb wirken gegen die Exzitotoxizit\u00e4t (nervensch\u00e4digende \u00dcbererregung) durch den\u00a0Hirnbotenstoff Glutamat, wirken zudem antioxidativ, entz\u00fcndungshemmend und neuroprotektiv. Ketamin (Ketalar<sup>\u00ae<\/sup>), Esketamin (Spravato<sup>\u00ae<\/sup>), Memantin (Axura<sup>\u00ae<\/sup>, Ebixa<sup>\u00ae<\/sup>), Riluzol (Rilutek<sup>\u00ae<\/sup>), D-Cycloserin (Seromycin<sup>\u00ae<\/sup>), Minocyclin (Minocin<sup>\u00ae<\/sup>)\u00a0und andere Substanzen bzw. Medikamente\u00a0reduzieren\u00a0bzw.\u00a0verhindern die\u00a0Sch\u00e4digung der Hirnzellen durch Glutamat. Diese Medikamente sind Alternativen zu den Antidepressiva bei Therapieresistenz der Depression.<\/span><\/p>\r\n<p><span style=\"font-family: 'Open Sans';\">\r\n\r\n<\/span><\/p>\r\n<h3
